The closest to a check list is the Doctoral Dissertation Guide and Thesis Formatting webpages that can be found on our Thesis and Dissertation Guide webpage. The requirements for the signature sheet are described on these webpages.
The dissertation should be printed on one side of the pages, and you are asked to submit both the physical copy as well as a pdf. Although I believe the pdf could be emailed instead of sacrificing a USB drive (see the Thesis Order Form for more info on this.)
